Health Cloud Partnership to Support Improved Risk Management, Care Quality

Franciscan Health is leveraging the Innovaccer Platform to enhance its risk-based contract performance and advance patient-centered care.

- Franciscan Health and Innovaccer, Inc., announced a partnership under which the 12-hospital system with facilities in Indiana and Illinois will leverage the company's advanced analytics to support its value-based care efforts.

According to a press release shared with HealthITAnalytics, Franciscan Health will use the Innovaccer Health Cloud’s Data Activation Platform to integrate patient records across its network and affiliated providers to advance the health system’s population health and value-based care strategies. The health system is also set to utilize the Innovaccer Health Cloud’s Customer Relationship Management (CRM) offering to enhance consumer and patient experience strategy.

Using the platform, Franciscan Health will be able to integrate all its data into unified patient records, allowing the health system to use advanced analytics to identify indicators related to risk management and care quality. The press release further states that Franciscan Health will use these tools to stratify patient populations based on risk, which could help improve its Medicare Risk Adjustment (RAF) and quality scores, reduce readmissions, and improve efficiency.

“Innovaccer brings together all of the data that exists in various information technology silos across our health system, giving us a true enterprise-wide unified patient record,” said Randy Moore, MD, chief operating officer at Franciscan Health, in the press release. “We can leverage that unified patient record using Innovaccer’s population health and value-based care solutions to proactively identify patients most in need of an intervention, and give our care teams the analytics and seamless workflows they need to support coordinated care efforts. In the future, the Innovaccer Health Cloud can be used to support strategies such as reducing sepsis infections and accelerating meaningful reductions in readmissions.”

These efforts build on an existing collaboration between Innovaccer and Franciscan Health aimed at using unified patient records to support whole-person care. Whole-person care allows providers to treat patients by looking at their full range of needs, including medical, behavioral health, and socioeconomic factors.

Franciscan Health used Innovaccer’s Health Cloud offerings to bridge these “data islands,” which separate data across systems, locations, and care settings, the press release notes. By unifying these data, the health system aims to create a single source of truth about a patient.

“Bringing data together from across multiple systems and physical locations is very challenging,” Moore said. “But Innovaccer makes that integration possible, and quickly provides a unified patient record at the point of care that can help us drive better care protocols, improve care management and coordination, and boost efficiency to improve both clinical and business outcomes. Further, the health system will use these unified patient records to support enhanced relationship management and patient engagement. According to the press release, this will allow Franciscan Health to create personalized care paths for patients and engage high-risk patients through calls, text, chat, and email in an effort to advance value-based care.
