Community Health Systems Deploys AI to Improve Remote Patient Monitoring

Community Health Systems will use Candence technology to enhance remote patient monitoring practices, promoting chronic disease management.

- Community Health Systems and Cadence announced a partnership to improve remote patient monitoring for those with chronic conditions.

By collecting and analyzing patient vitals, medical history, and health data from their homes, Candence creates personalized, real-time treatment responses to improve chronic disease management.

The CHS-Candence partnership will initially support individuals managing hypertension, heart failure, diabetes, and chronic obstructive pulmonary disease. However, as the partnership grows, Cadence will assist in expanding CHS’s remote care platform to support an increasing number of conditions.

“Our partnership with Cadence offers powerful technology and virtual care to support patients in the prevention, monitoring, and management of chronic conditions,” President of Clinical Operations and Chief Medical Officer of Community Health Systems Lynn Simon, MD, said in a press release. “Daily and continuous monitoring at home can improve a patient’s overall health and outcomes and also enable early interventions when necessary.”

Remote patient monitoring can also lead to fewer preventable hospitalizations, a reduction in the cost of care for specific patient populations, and enhanced patient engagement and satisfaction.

According to the Centers for Disease Control and Prevention (CDC), six in 10 adults living in the United States have a chronic disease and four in 10 have two or more chronic diseases. Chronic diseases are one of the leading causes of death and disability in the US and significantly drive-up healthcare costs.

Remote patient monitoring allows those with chronic conditions to access health services from their home. As this delivery approach grows in the healthcare community, more hospital systems and organizations are partnering with health technology companies.

Earlier this month, UCI Health announced a partnership with Biofourmis to expand its remote patient monitoring capabilities through AI, which is an important technique for chronic disease management.
